This badge is available to everyone and can be equipped by anyone watching a stream without sound, for example because of unreliable audio, a muted player, or hearing impairment. It is selected manually through the Chat Identity menu next to the chat input box, and can be enabled globally or only in specific channels.
It was added in May 2022 together with the matching "Watching without video" badge as part of Twitch's chat identity options. Wearing the badge is purely self-declared and does not verify that the viewer actually has their audio off.
